Turning cardiovascular disease from a terminal pathway into a preventable condition
EXIT-CVD is a five-year, €27.6 million public–private partnership supported by the Innovative Health Initiative Joint Undertaking — uniting 42 organisations across 17 countries to find strategic “exit points” where disease progression can be halted or reversed.
Background
Why cardiovascular care needs a new exit strategy
Heart disease rarely appears overnight. It builds across decades — yet most systems still respond only after damage is done. EXIT-CVD was launched to intervene earlier, smarter, and more equitably along that continuum.
By aligning prevention, stratified treatment, and advanced recovery within one programme, the consortium tests whether targeted “exit points” can slow or reverse progression before patients reach irreversible stages.
The project sits within the EU’s push for personalised, data-driven cardiovascular care — supported by the Innovative Health Initiative and aligned with European Society of Cardiology priorities on prevention, early detection, and integrated care pathways across the disease continuum.
Challenges
Barriers we must address together
Cardiovascular disease remains Europe's leading cause of death. Despite advances in treatment, three structural barriers still prevent patients from leaving the disease pathway in time.
Most health systems are built around acute events — heart attacks, strokes, and hospital admissions — rather than long-term risk reduction. High-risk individuals often receive little structured support until irreversible damage has occurred. EXIT-CVD reframes care around identifiable moments earlier in the continuum where intervention can still change outcomes.
Imaging, wearables, biomarkers, and electronic health records rarely flow into a single, actionable picture of cardiovascular risk. Without harmonised data and AI-ready infrastructure, clinicians cannot reliably stratify patients or deploy precision tools at scale. The consortium builds federated systems that connect sites without compromising privacy or equity.
New diagnostics and digital tools often reach well-resourced populations first, widening gaps in access and outcomes. EXIT-CVD embeds equity-by-design across trials, technology development, and deployment — so exit strategies are validated in diverse communities and health settings, not only in tertiary centres.

Where EXIT-CVD is heading
A future where cardiovascular disease is detected early, treated precisely, and — where possible — reversed before advanced heart failure becomes inevitable. EXIT-CVD envisions a continent-wide model of care built on evidence-based exit points at every stage of the disease continuum.
Validate early, mid, and late exit strategies through 22 clinical studies; develop AI-assisted tools for risk stratification and decision support; harmonise data across 42 partner sites; and deliver guidelines, training, and implementation pathways that health systems can adopt.
Shorter time to diagnosis, fairer access to innovation, and robust evidence for clinicians, patients, and policymakers. By 2031, EXIT-CVD aims to show measurable gains in prevention uptake, personalised treatment, and recovery outcomes — with tools and models ready for scale-up beyond the consortium.
