Study overview
Ischemic heart disease remains the leading cause of morbidity and mortality worldwide. Despite major advances in medical therapy and lifestyle interventions, sustained control of cardiovascular risk factors (CVRFs) and treatment adherence remain significant challenges. This study aims to improve CVRF control after acute coronary syndrome (ACS) by implementing a structured monitoring smartphone app-based program that integrates early patient education, continuous wearable-based monitoring, and adherence-promoting strategies throughout the first year following ACS.
Objectives
Primary
To evaluate whether a structured, early post-discharge monitoring smartphone app-based program using wearable devices improves CVRF control, treatment adherence and patient satisfaction.
Secondary
- To reduce unplanned hospital visits by enabling prompt clinical response to medication-related adverse effects.
- To support timely reintegration into the workforce for employed patients through a structured program integrating psychological support.
- To assess patient adherence to lifestyle interventions (diet, physical activity, medication).
- To determine whether continuous monitoring facilitates the early detection of uncontrolled CVRFs (i.e., hypertension) and/or undiagnosed comorbidities (i.e., electrical rhythm disturbances).
- To evaluate the feasibility of continuous monitoring strategies in the post-ACS setting and the degree of patient satisfaction.
